There are at least two versions of this flat type. Both feature the words "Combat Wounded" atop purple heart image along the left side of numbers with stacked P/H stacked along right.
First older version has a mostly purple left side logo and text with number and stacked P/H but NO bottom caption, as on personalized "TJM 518" observed by Jim Sadler.
Second newer version illustrated here has a slightly different logo design and state name font plus the "PURPLE HEART" bottom caption.
